These are some of the forms required in order for us to work together.  Depending on how we will be meeting, you may be asked to complete an additional for to consent for Telehealth treatment. 

 

When we speak and set up an initial appointment, I will ask for your email and then send you a link to my online portal (Jituzu).  Then, I will email you directly all forms and you can complete them and return them through a secure platform.  Through this portal, you can schedule/ reschedule your appointments, email me, pay your bill and share other confidential forms and papers.
